Subacromial Impingement Syndrome (SAIS) is the commonest disorder of the shoulder, accounting for 44%–65% of all complaints of shoulder pain. 1 In a Dutch study, the incidence of new cases of rotator cuff tendonitis in general practice was found to be around 3.2–4.2 per 1,000 person-years, and the corresponding incidence of shoulder pain (all causes) was 11.2 per 1,000 person 2013-10-26 for subacromial impingement syndrome: a nationwide Danish cohort study The adjusted OR (ORadj) increased to a maximum of 2.1 for arm-elevation-years, repetition-years and force-years. What is a subacromial impingement ? From the age of 40, pain in the shoulder is often caused by what is called subacromial impingement, a syndrome linked to the aging of the rotator cuff tendons. The muscles and tendons surrounding the head of the humerus make up the rotator cuffs, part of the shoulder joint, which enable lifting and rotation of the arm, and help stabilise the shoulder.

Subacromial impingement occurs when the subacromial space is reduced and which causes the humeral head, when elevated, to move closer than normal together to the acromion. There are many different types of reasons or pathologies for this condition. Subacromial impingement occurs when the tendons of the shoulder joint (or rotator cuff) are injured as they glide against the bones and ligaments of the shoulder. More specifically, the rotator cuff tendons can rub against the acromion process during overhead activities which can lead to pain and degeneration of the tendons.

Subacromial impingement is by far the most common form of shoulder impingement and occurs secondary to attrition between the coracoacromial arch and the underlying supraspinatus tendon or subacromial bursa, leading to tendinopathy and bursitis respectively.

Shoulder impingement syndrome is a syndrome involving tendonitis of the rotator cuff muscles as they pass through the subacromial space, the passage beneath the acromion. It is particularly associated with tendonitis of the supraspinatus muscle. This can result in pain, weakness, and loss of movement at the shoulder.
